Dr. Cyril Wecht has revealed to PEOPLE that according to the toxicology report...
a lethal combination of methadone, Zoloft and Lexapro which caused a cardiac dysrhythmia
So basically a poor combination of medicines made Daniel Smith stop breathing and he died.
Dr. Cyril Wecht got the toxicology results from National Medical Services in Willow Grove, Pa., on Wednesday.

"In my opinion it's an accidental death,"
Dr. Cyril Wecht Wecht said that because Daniel Smith was taking three drugs and not a large dose of a single drug, paired with the fact that he was celebrating a happy occasion with his mother, those factors point to a
"tragic accidental drug death."
Dr Cyril Wecht was asked why Daniel Smith may have been taking methadone, which is sometimes used to help heroin addicts end their addiction, Dr Cyril Wecht said the drug is
"a legitimate prescription drug for pain relief"
and that Daniel Smith had
"no known history of morphine addiction."
It is just a real shame and a waste of life...
